• meet you and/or your family in your home or
agreed upon location.
• empower you and/or your family to achieve as much independence as
possible in your desired setting.
• advocate for you and support you in order that you achieve your goals and age with dignity.
• help you and your family "create order out of chaos".
• teach you how to maximize your life experiences and your skills to create a safe environment to ensure your well-being.
• Massachusetts Medicaid applications will be initiated as appropriate on an individual basis.
• telephone consultation will be available as appropriate.
• payment will be expected at time of visit/consultation.
